Find the right licence record
Start with the type of licence, then match the full name and licence number. The links below go to official resources.
Medical doctor search
Arizona’s Medical Board search is for medical doctors, whose licence says MD. It also links to the board’s written decisions on a doctor’s licence, known as board orders.
Ask the board to explain any penalty or limit on the doctor’s work listed in a decision.
Doctor with a DO licence
DO is another type of doctor’s licence. The Osteopathic Board is Arizona’s separate licence board for these doctors.
Nursing licence checks
Nurse practitioners are nurses who have extra training for making care decisions.
The Nursing Board has links for the advanced-practice certificate and the nursing licence. The certificate records that extra nursing role.
In Arizona, prescribing medicine also needs the board’s permission for the patient group the nurse is trained to treat.
Ask the board to find the nurse practitioner’s advanced-practice certificate. Ask whether it covers people like you and includes permission to prescribe the medicine being offered.
Check a PA licence
PA stands for physician assistant, a health professional who gives medical care with a PA licence. This Arizona board has the search for that licence.
Ask the board whether the PA’s licence allows the care and prescription being offered.
Find the state pharmacy check
The FDA, the US Food and Drug Administration, links to state pharmacy licence searches from this page.
Choose Arizona, then look for the pharmacy that will send the medicine.
October 3, 2026 check of Arizona’s pharmacy rules.
Checking a pharmacist’s licence and checking the pharmacy’s permit are different tasks.
A pharmacy based outside Arizona needs this state’s permit to send medicines here. A permit is state permission. The pharmacy also needs its home state’s permission to operate.

FDA’s compounding explanation
Compounding is preparing a medicine to order.
Made-to-order medicines have no approval from the FDA as of September 26, 2026. The agency does not review them before patients receive them.
Read this when a made-to-order version is offered. Ask why that version is being proposed for you.
FDA’s guide to online pharmacies
The FDA lists a valid prescription, a state licence and access to a licensed pharmacist among its checks.
It also cautions that a convincing website can hide a fake pharmacy.
Use the official record as well as the website’s appearance.
FDA’s weight-medicine concerns
The FDA says compounding retatrutide or cagrilintide is against federal law as of September 26, 2026.
These are medicine names. Ask about their status if either name appears in an offer.
Check a care or payment question
Arizona’s remote-care requirements
An in-person visit is not required for every prescription under Arizona’s rules, checked October 3, 2026. The clinician still has to examine you in a way that suits your health needs and follow the rules for that medicine.
Ask the clinician whether that examination can happen by screen or needs a visit in the same room.
Medicare’s weight-drug information
Eligible members could get certain weight-loss medicines for $50 per monthly supply through a temporary Medicare programme as of September 26, 2026.
Use this to ask whether you qualify, then ask about your own plan and health situation.
How to report a medicine problem
MedWatch lets patients send medicine-problem reports to the FDA without a fee.
Use the reporting information after arranging any care the problem needs.
